1. Obtaining a Duplicate Title
  • Vehicle Owners - If you own your vehicle and the lien on it has been satisfied and you need to request a duplicate title, you will need to complete form MV-38O. Please mail the completed form to PennDOT at the address listed on the form along with a check or money order made payable to the Commonwealth of Pennsylvania.  Please refer to MV-70S, "Bureau of Motor Vehicles Schedule of Fees," for required fees.
  • Lienholders - For lienholders who need to request a duplicate title or make changes to an existing title, please complete form MV-38L. Please mail the completed form to PennDOT at the address listed on the form, along with a check or money order for the appropriate fee made payable to the Commonwealth of Pennsylvania.  Please refer to MV-70S, "Bureau of Motor Vehicles Schedule of Fees," for required fees.
  • Dealers - Registered dealers who need to apply for a duplicate title should complete form MV-38D. Please mail the completed form to PennDOT at the address listed on the form, along with a check or money order made payable to the Commonwealth of Pennsylvania.  Please refer to MV-70S, "Bureau of Motor Vehicles Schedule of Fees," for required fees.
  • Defaced Titles - If your title has been defaced, you will need to apply for a duplicate title. For more information on what constitutes a defaced title, please refer to the Defaced Title Fact Sheet.

2. Issue with Printing Registration Renewal Card Online
If you renewed your registration online and an error occurred that would not allow you to print a permanent registration credential at the time of the online registration transaction, you may use our website and request a free reprint. No fees are required if it is completed online through PennDOT's publicly accessible website.

3. Obtaining a Duplicate Person with Disability Parking Placard or Person with Disability Registration Plate
  • How to Apply for a Replacement Person with Disability Placard - You may apply for a replacement Person with Disability Parking Placard by completing Form MV-145A “Person with Disability Parking Placard Application.” Your previous placard number will be listed as the previous placard on PennDOT’s computer system. If the placard is ever located, it must be returned to PennDOT because it will now be null and void.

    You may renew, request a replacement and/or complete a change of address for your Persons with Disability Placard online.

    NOTE: If the replacement request is for a temporary placard, the placard will be issued for the balance of the time remaining on the original temporary placard issued.

  • Apply for a Replacement Person with Disability Registration Plate - You may apply for a replacement Person with Disability Registration Plate by completing Form MV-44 (PDF), “Application for Duplicate Registration Card or Replacement of Registration Plate,” indicating the reason for the replacement. Please mail the completed form to PennDOT at the address listed on the form along with a check or money order for the appropriate fee(s) and made payable to the Commonwealth of Pennsylvania. Please refer to Form MV-70s, “Bureau of Motor Vehicles Schedule of Fees,” for the required fees.

  •  Apply for a Replacement Person with Disability Motorcycle Plate - You may apply for a replacement Person with Disability Motorcycle Registration Plate by completing Form MV-44 (PDF), “Application for Duplicate Registration Card or Replacement of Registration Plate,” indicating the reason for the replacement. Please mail the completed form to PennDOT at the address listed on the form along with a check or money order for the appropriate fee(s) and made payable to the Commonwealth of Pennsylvania. Please refer to Form MV-70s, “Bureau of Motor Vehicles Schedule of Fees,” for the required fees.
